This policy explains how personal and health information is collected, used, stored and disclosed, in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles, and South Australian health records requirements.

What is collected. Contact details, Medicare and referral information, and health information relevant to your care, including session notes, assessments and correspondence. For NDIS participants, plan and funding details needed for invoicing.

How it is collected. Directly from you through booking, intake forms and sessions, and with your consent from your GP, referrers or others involved in your care.

Why it is collected. To provide psychological services, communicate with you, process Medicare, NDIS and private payments, and meet legal and professional obligations.

Storage and security. Records are held in Cliniko, a practice management system that stores data on servers located in Australia, protected by encryption and access controls. Telehealth sessions are conducted over encrypted video and are not recorded.

Disclosure. What you share is confidential. Information is disclosed only with your consent (for example, reports to your GP), or where required or permitted by law, including where there is a serious risk to your safety or someone else's, or where records are subpoenaed. These limits are explained in plain language at your first session.

Access and correction. You may request access to, or correction of, your information at any time by emailing hello@tristanduggan.com.au. Requests are handled in accordance with the Privacy Act; access may be declined in limited circumstances permitted by law, with reasons provided.

Retention. Health records are retained for a minimum of 7 years after your last contact, or for a young person, until age 25, whichever is longer, then securely destroyed.

Complaints. Raise any privacy concern with me directly at hello@tristanduggan.com.au. If unresolved, you may contact the Office of the Australian Information Commissioner (oaic.gov.au, 1300 363 992).
